Minimum Requirements

  • Master's or Doctoral Degree in Physical Therapy from an accredited program.
  • Active Texas Physical Therapist license required prior to start.
  • BLS certification from AHA or ARC may be required.
  • Home health and HCHB experience required.
  • Experience with home health documentation: SOCs, ROCs, recertifications/reassessments, and plan of care development.
  • Experience supervising PTAs.
  • Preferred: orthopedics and neuro experience.


Assignment Details

  • Facility Type: Home Health.
  • Shift Name: Days.
  • Shift Details: Monday–Friday, 08:00–16:00.
  • Weekly Hours: 40.
  • Productivity Expectation: 30 points.
  • Coverage Areas: Hawkins, Mineola, Lindale (local travel between patient visits required). Mileage reimbursed between visits from first to last patient.
  • Primary responsibilities: patient assessment, complete plan of care for home health, SOCs, ROCs, recerts/reassessments, supervise PTA.
  • Dress code: scrubs in black, gray, or purple.
