Travel Physical Therapist (PT) - $2,224 per week in Webster, NY

Job Title: Physical Therapist Profession: Physical Therapist Specialty: Occupational Therapist Duration: 14 weeks Shift: 5x8 Hours per Shift: 8 hours Experience: Minimum of one year experience in Long Term Care/Home Care setting as Physical Therapy License: Possession of a current Client License in Physical Therapy Certifications: BLS (AHA ONLY) Must-Have: Oasis experience, Home Health Experience within the last year Description: Provides case management and direct patient care within a team environment. Administers comprehensive physical therapy modalities in the home setting. Assists the physician in evaluating the level of function and helps develop the plan of care, revising it as necessary. Prepares clinical and progress notes and consults with families and other personnel. Participates in in-service programs and promotes quality care through effective relationships with patients, families, and staff. Utilizes resources efficiently to contain costs and increase the value of services to the community. Promotes positive, supportive, and respectful communication to all external sources, patients, families, and staff. Develops and revises treatment and discharge plans in conjunction with patients, families, and physicians. Provides an environment that promotes respect for patients' privacy, confidentiality, and property. Initiates appropriate rehabilitative programs and supervises physical therapy assistants to ensure patient health and safety. Implements home therapy programs, including gait training, transfers, and activities of daily living. Periodically evaluates patient progress and makes necessary adjustments to therapy programs. Instructs para-professional staff regarding body mechanics, transfers, and home exercise programs. Oversees the quality of patient care services provided and interacts with patients and medical personnel in a positive manner. Supports collaborative planning and case management that meets individual health and service needs. Participates in regularly scheduled patient conferences to discuss patient care and family problems. Assists in the orientation of new rehabilitation personnel as needed. Ensures the completion of all clinical records needed for compliance with relevant legislation. Refers to appropriate community resources and maintains knowledge of available services. Demonstrates flexibility, enthusiasm, and willingness to cooperate with others. Makes constructive recommendations for improving patient care and operational processes. Ensures patient needs are met throughout the care delivery process. Maintains compliance with agency performance competencies and participates in achieving operational objectives.
