Description:
Monday - Friday 8:00 AM - 4:30 PM; weekends as needed.
Mileage can be reimbursed.
Provides case management and direct patient care within a team environment.
Administers comprehensive physical therapy modalities in the home setting.
The qualified therapist assists the physician in evaluating the level of function.
Helps develop the plan of care and revises it as necessary.
Prepares clinical and progress notes.
Advises and consults with the family and other personnel.
Participates in in-service programs.
Participates to ensure that care delivery services are consistent with the agency's vision, mission, and values.
Promotes effective working relationships and works effectively as part of the team to meet goals and objectives.
Promotes a positive image by adhering to the dress code policy.
Utilizes resources, supplies, and time efficiently to contain costs and enhance service value.
Promotes positive, supportive, respectful communication to all external sources, patients, families, and staff.

Supports public relations and marketing initiatives to the local community.

Direct patient care duties include developing and revising treatment and discharge plans in conjunction with patients.

Initiates appropriate rehabilitative programs.
Provides supervision of assistants to ensure patient health and compliance with plans of care.
Consults with and educates patients and families regarding disease processes and self-care techniques.
Implements home therapy programs including gait training, transfers, activities of daily living, and home modifications.
Periodically evaluates patient progress and makes necessary adjustments to the therapy program.
Instructs staff in body mechanics, equipment use, and home exercise programs.
Directs, coordinates, evaluates, and supervises quality of care provided.
Interacts with all stakeholders in a manner conducive to maintaining positive relationships.
Displays patience, courtesy, and tact in all interactions.
Supports a collaborative customer-focused planning and case management approach.
Participates in patient conferences with teams to discuss patient care and to ensure comprehensive plans.
Assists in the orientation of new rehabilitation personnel.
Performs other duties to support the efficient operation of the team.
Ensures completion of clinical records for compliance with legislation.
Refers to appropriate community resources.
Maintains knowledge of available services.
