Minimum Requirements

• Associate's, Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Minimum education requirement: Associate Degree (client specific).
• Active Massachusetts Physical Therapy license required to start the assignment.
• Home health experience required. Oasis and homecare experience required (client specific).
• Experience using OASIS documentation and homecare workflows (client specific).
• BLS certification from AHA or ARC may be required.
• Interpersonal skills preferred.

Assignment Details

• Facility Type: Home Health.
• Shift Name: Days.
• Shift Details: Days 0800-1600. Typical scheduled hours noted as 8:00 AM to 4:30 PM in client notes.
• Weekly hours: Guarantee of 32 hours with availability of up to 40 hours based on caseload (client specific).
• Productivity expectations: 26 points in 32 hours or 33 points in 40 hours (client specific).
• Coverage Area: Travel to patient homes in Shrewsbury, Westborough, Northborough, Bolton, Hudson, Marlborough, and Framingham service areas (client specific). Mileage reimbursed between visits from first to last patient.
• On-call/Weekend Requirement: No regular on-call requirement. Occasional day swaps may be permitted for valid reasons (client specific).
