Key Responsibilities of the Physical Therapist- Perform physical therapy evaluation and treatment based on physician orders/referrals
- Conduct comprehensive evaluations using appropriate tools and procedures
- Establish measurable goals and develop treatment plans
- Regularly reassess treatment effectiveness and modify as needed
- Communicate patient needs and progress to treatment team and others involved
- Collaborate with medical providers on patient progress and discharge plans
- Recommend and assist with adaptive equipment to enhance patient function
- Maintain documentation as required by regulations and company standards
- Adhere to professional practice standards and legal requirements
- Supervise Physical Therapist Assistants and Aides in patient care activities
- Participate in staff development programs and continue clinical education
Requirements of the Physical Therapist- Compact license in RI
- Experience with HCHB Homecare, home exercise planning, and Oasis
