Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Current Physical Therapy license in Washington state or a valid compact license granting privilege to practice in Washington.
- Current BLS/CPR certification from AHA or ARC.
- Valid driver’s license, current vehicle insurance, and access to a dependable vehicle.
- Home health experience preferred.
- Experience with HomeCare HomeBase preferred.
- Ability to complete patient evaluations and develop plan of care within state-specific guidelines.
- Ability to travel and visit patients across South King County service area. May be required to visit other nearby agency service areas as needed.
- Must be able to participate in occasional weekend rotation and potential availability on observed holidays as needed. (Client indicates schedule flexibility required.)

- Shift hours: 08:00–16:00 (M–F; client also notes M–F 8:00–4:30 schedule and a 36-hour weekly guarantee).

- Weekend/on-call: No routine on-call requirement; schedule flexibility and participation in a weekend rotation may be required per client guidance.
- Mileage reimbursed between visits from first patient to last patient.
- May supervise PTAs and home health aides per agency policy and state regulation.

Job Responsibilities
- Evaluate patients’ functional status and physical therapy needs and consult with physicians to develop plans of care in accordance with CMS documentation requirements.
- Provide physical therapy services within state scope of practice, following the plan of care and coordinating with the interdisciplinary team.
- Document progress notes per agency policy and complete patient evaluations and plan of care within state timelines. Report evaluation outcomes to nurse supervisor within 24 hours.
- Treat patients to relieve pain, restore or maintain function using evidence-based techniques and acceptable standards of practice.
- Educate patients, families, and caregivers regarding the plan of care, functional limitations, and progress toward goals.
- Participate in staff meetings, committees, and provide in-service education as requested by agency leadership.
- Prepare written discharge summaries and progress reports at least every 60 days, and perform other duties as assigned.
Professional and Compliance Notes
- Adhere to agency Therapy Standards of Practice and maintain state licensure and required certifications through continuing education.
- Follow agency policies for supervision of PTAs and home health aides in accordance with state regulations.
