Description:
We are seeking a licensed Physical Therapist (PT) to provide in-home physical therapy services.
The Physical Therapist will deliver skilled physical therapy services in patients' homes in accordance with physician orders, payer requirements, and applicable state and federal regulations.
This role supports patient mobility, safety, and functional independence through individualized, evidence-based care.
Clinical Responsibilities:
Perform assigned physical therapy visits, including evaluations, Start of Care (SOC), reassessments, recertifications, and discharge visits.
Develop and implement individualized physical therapy plans of care based on patient needs and physician orders.
Provide skilled interventions such as gait training, balance training, therapeutic exercise, mobility training, and fall prevention.
Educate patients and caregivers on treatment plans, home exercise programs, and safety strategies.
Communicate patient progress, concerns, or changes in condition to the interdisciplinary care team.
Collaborate in the clinical oversight of Physical Therapist Assistants (PTAs) as applicable and permitted under Colorado law and agency policy.
Documentation & Systems:
Complete all required documentation accurately and timely in Homecare Homebase (HCHB).
Ensure documentation complies with Medicare Conditions of Participation, payer requirements, and agency standards.
Maintain compliance with HIPAA and all applicable privacy and security requirements.
Care Coordination:
Collaborate with clinical staff, including RNs, PTAs (when applicable), OTs, STs, and leadership.
Participate in care coordination related to patient needs, visit completion, and regulatory requirements.
Provide clinical oversight of PTAs only when applicable, assigned, and permitted under Colorado law and agency policy.
Practice Environment:
Patient-centered, in-home care with one-on-one visits.
