Key Responsibilities of the Physical Therapist- Perform physical therapy evaluations and treatments based on physician orders/referrals
- Complete comprehensive evaluations using appropriate tools and procedures
- Review patient records for diagnosis, history, precautions, and contraindications
- Establish measurable goals and develop treatment plans for patient independence
- Re-assess treatment plans regularly and communicate progress with the team
- Recommend adaptive equipment and integrate into rehab programs
- Maintain and submit documentation as required by regulations
- Supervise Physical Therapist Assistants and Aides
- Participate in staff development and maintain clinical practice skills
Requirements of the Physical Therapist- Physical Therapy License in NJ
- Experience with Home care home base (HCHB) and other care systems
- Strong organizational skills and proficiency in Microsoft Teams
