Job Title: Physical Therapist Profession: Therapy/Rehabilitation Specialty: PT Home Health Duration: 13 Weeks Shift: Day 5x8-Hour Hours per Shift: 08:00 - 17:00 Experience: Minimum 1 full year of Adult Home Health Experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Recent Home Health experience Description: The position involves providing home health services which include home visits and periodic office meetings. Candidates must have reliable transportation, a current driver's license, and agency-required liability insurance. Average number of patients per day is between 5 to 10, which equates to roughly 6-8 points based on visit types and activities, including documentation and travel. There is no guaranteed hours; if a situation arises where the clinician cannot meet financial needs due to a low patient census, discussions with management are encouraged for potential floating to another clinic. Types of cases include Start of Care and Follow Ups. Weekend requirements may vary by location and profession and should be discussed during the interview. Travel to patient homes may extend up to 75 miles from the assigned clinic, and mileage is reimbursed at the IRS standard rate. Documentation responsibilities must be completed within 24 hours of the appointment with the patient. All travelers are expected to be proficient in home health and experience with OASIS visits/scoring and home health electronic medical record systems. Candidates are expected to accept every patient on their schedule, regardless of the distance to travel from the previous appointment. Delayed or incomplete documentation that impacts billing and patient care will be addressed. Clinicians should be prepared for potential on-call rotations and must be open to travel expectations as outlined. This opportunity requires a commitment to productivity points, equating to 40 hours per week to meet expectations.
