Key Responsibilities of the Licensed Physical Therapist

• Perform physical therapy evaluation and treatment based on physician orders/referrals
• Complete comprehensive evaluations using appropriate tools and procedures
• Review patient records for diagnosis, medical history, and contraindications
• Establish measurable goals and develop treatment plans for independence and function
• Regularly re-assess treatment effectiveness and patient progress
• Communicate patient needs and progress with the treatment team and family
• Consult with medical providers about patient evaluation and treatment
• Recommend and assist with securing adaptive equipment for patients
• Maintain and submit required documentation
• Adhere to Company policies, HIPAA, and professional practice standards
• Supervise Physical Therapist Assistants and Aides in patient care
• Participate in staff development programs and obtain continuing education

Requirements of the Licensed Physical Therapist

• Valid Physical Therapist license in Maryland
• Experience with Home Care Home Base preferred
