Travel Physical Therapist (PT) - $2,486 per week in Sioux City, IA

Job Title: Physical Therapist Profession: Physical Therapist Specialty: Outpatient Duration: 13 weeks Shift: Monday - Thursday or Monday - Friday Hours per Shift: 8:00 AM - 4:30 PM Experience: 1 year of previous experience required; at least 1 travel contract completed preferred License: State license required Certifications: BLS required Must-Have: - Driver's License required - Work history/resume required - Skills checklist completed within 1 year of submission date required - License verification required - Copy of required certifications preferred - 1 supervisory reference required at the time of submission - Must have a taxable address outside of 50 miles from the facility Description: As a member of the interdisciplinary team, the traveler contributes professional and clinical knowledge and skills in the provision and management of care to patients. The traveler may be asked to float within the same discipline across local campuses. The traveler will be required to work 1-2 of the following summer holidays: Memorial Day, Independence Day, Labor Day. Candidates should not have been an employee of this organization in the last 12 months. Epic experience preferred. References must include 1 supervisory reference at the time of submission.
