Key Responsibilities of the Physical Therapist- Perform physical therapy evaluations and treatments based on physician orders/referrals
- Complete comprehensive evaluations using appropriate tools and procedures
- Review patient records for current diagnosis and history
- Develop treatment plans to maximize patient independence and function
- Regularly reassess treatment plan effectiveness and patient progress
- Communicate patient needs and progress to the treatment team and family members
- Consult with medical providers regarding patient evaluation, progress, and discharge
- Recommend and assist in securing adaptive equipment for patients
- Maintain and submit required documentation
- Supervise Physical Therapist Assistants and Aides
- Participate in staff development programs and maintain clinical skills
Requirements of the Physical Therapist- Compact license RI
- Experience with HCHB Homecare
- Skills in home exercise planning and Oasis
