Travel Physical Therapist (PT) - $2,479 per week in Haverhill, MA

Job Title: Physical Therapist Profession: Physical Therapist (PT) Specialty: Long Term Care (LTC) Duration: 13 weeks Shift: Days Hours per Shift: 8 hours Experience: 1 year of Physical Therapy experience or equivalent Level II experience License: Massachusetts State License Description: Effectively screens, evaluates, and rehabilitates patients with physical functioning disorders and/or wound care. Develops an appropriate plan of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practice to support clinical interventions. Administers various procedures as part of the rehabilitation plan including manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive and assistive devices. Assesses patients' progress and adjusts treatments accordingly in the clinical record per regulatory and clinical practice requirements. Evaluates treatment outcomes for effectiveness. Modifies services in relation to patient evaluations and makes referrals based on outcomes. Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders. Effectively communicates with supervisors and other health team members regarding patient progress, barriers, and treatment plans. Provides a comprehensive discharge summary of services provided per regulatory and clinical requirements. Documents all required information in the electronic documentation system and maintains patient records according to policy. Accurately records treatment minutes for services provided per all regulatory and clinical requirements. Participates in required meetings including patient care conferences, utilization review meetings, and rehabilitation meetings. Engages in facility and company-required training, in-services, and conferences. Able to obtain any necessary durable medical equipment or supplies per regulatory and clinical practice requirements. Able to supervise and support physical therapist assistants, rehabilitation aides, and students. Adheres to required practice acts, code of conduct, and ethics during professional practice as a Physical Therapist.
