Travel Physical Therapist (PT) - $2,492 per week in Lake Zurich, IL

Job Title: Physical Therapist Profession: Therapy/Rehabilitation Specialty: Home Health Duration: 13 Weeks Shift: Day Hours per Shift: 8 Hours (08:00 - 17:00) Experience: Minimum 1 full year Home Health experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Reliable transportation, current driver's license, agency-required liability insurance Description: Home visits will be required for this position. Periodic office meetings will be scheduled. The contractor must have reliable transportation. A current driver's license and agency-required liability insurance are necessary. On average, a contractor will see 5-6 patients per day. Contractors will be assigned between 5-10 patients per day. This equates to roughly 6-8 points of productivity per day. Types of cases include Start of Care and Follow-Up visits. Weekend requirements may vary based on location and profession. All candidates must have recent Home Health experience. Traveling up to 75 miles from home is required. Mileage will be reimbursed at the IRS standard rate. The commute to and from the first and last patient is not billable. This position is open to local candidates at the same rate. All documentation must be completed within 24 hours of the appointment. Candidates will accept all patients on their schedule, regardless of distance. There are no guaranteed hours with this contract. Weekly productivity points of 30 must be achieved, equating to approximately 40 hours per week. If there are not enough patients, the contractor may float to another nearby clinic. Experience in HomeCare HomeBase EMR is required. Candidates must be proficient in Home Health practices.
