Job Title: Physical Therapist Profession: Physical Therapist Specialty: - Duration: 5-month assignment, open to temp to perm Shift: M-F Hours per Shift: 40 Experience: Five years of progressively responsible PT experience; Minimum of two years Physical Therapy experience in a clinical setting with a frail or elderly population. License: Valid California Physical Therapy License Certifications: BLS Must-Have: - Physical Therapy knowledge and skills necessary to treat frail, elderly participants with complex needs. - Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency situations. - Two years supervisory experience with demonstrated ability to mentor and develop team members. - Excellent leadership, organizational, and communication skills in settings with seniors and interdisciplinary team members. - Experience leading in a data-driven organization, leveraging reports and data to prioritize and manage people and projects. - Ability to work independently with minimal supervision. - Demonstrated ability to prioritize in a fast-paced environment. - Experience and competency working with people from diverse backgrounds and cultures. - Commitment to unlocking the full potential of vulnerable seniors. Description: The Physical Therapist is responsible for coordinating and providing restorative and rehabilitative Physical Therapy services. This role involves working closely with the Physician, rehabilitation staff, and other interdisciplinary team members. The aim is to maximize participant independence and safety while developing or restoring function. The Physical Therapist collaborates with the Occupational Therapist to efficiently operate the rehabilitation department. This position is a member of the management team and facilitates collaborative relationships to promote values, team culture, and mission. Duties include conducting assessments to determine care needs, preferences, and goals of participants. Active participation in interdisciplinary team meetings to develop participant care plans is essential. The Physical Therapist will deliver and document interventions in accordance with physician orders and agreed care plans. These interventions include active and passive exercise, muscle re-education, gait, and functional training. Supervision and training of Therapy Assistants will be required for effective program implementation. Training and instruction to staff, participants, and caregivers in therapeutic equipment use and care is necessary. The Physical Therapist will also manage equipment orders and maintenance. Remaining alert to any changes in participants' condition and updating the interdisciplinary team promptly is critical. This position supports smooth care transitions between various settings. Participation in end-of-life care coordination and support is also a part of this role. Managing direct reports, including training and mentoring, is expected. All documentation must be completed in a timely and accurate manner. The Physical Therapist will protect privacy and maintain confidentiality of all company procedures and information. Following organizational policies, participating in quality improvement activities, and timely completion of mandated training is required. Communication with supervisors and teams regarding workload and priorities must be maintained. Flexibility in assignments as business needs evolve is essential. Additional duties may be assigned as necessary.
