Job Opportunity – Physical Therapist, Home Health – Medford, OR 97504

Start Date: October 5, 2026

Location: Medford, OR and surrounding areas

Schedule: Day shift, 8-hour shifts (08:00 - 16:30), Monday through Friday. This position does not accommodate 4x10 or 3x12 schedules. Typical caseload includes up to 6 patient visits per day with a standard of 30 productivity points. No on-call or weekend requirements.

Requirements:

• Active Oregon Physical Therapy license or compact license (must be in hand at time of submission)
• Minimum 1 year of skilled home health experience required
• OASIS documentation experience required
• Basic Life Support (BLS) certification
• COVID-19 primary vaccination series plus one booster required
• Reliable personal transportation for home visits (occasional travel up to 50+ mile radius)
• Two professional references covering 1 year within the last 3 years
