Key Responsibilities of the Physical Therapist

• Provide direct and indirect patient care services following approved care plans
• Evaluate patient needs and develop individualized treatment programs
• Administer complex therapeutic exercises
• Instruct, motivate, and assist patients and caregivers to improve functional mobility and daily living activities
• Offer clinical supervision and oversight of Physical Therapist Assistants and Home Health Aides
• Ensure appropriate use of adaptive equipment
• Maintain effective communication with the interdisciplinary care team

Requirements of the Physical Therapist

• PT license; compact licenses accepted
• Experience in home health
• Efficiency in Home Care Home Base (HCHB)
