Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment of patients based upon physician orders/referrals.
• Complete comprehensive evaluations using appropriate tools and procedures.
• Review patient records for current diagnosis, past medical history, precautions, and contraindications.
• Establish measurable goals and develop treatment plans to maximize patient independence and function.
• Regularly reassess treatment plans, patient progress, and modify goals or treatment as necessary.
• Communicate patient needs and progress to the treatment team and involved individuals.
• Consult with medical providers regarding patient evaluation, treatment, and discharge plans.
• Recommend and assist in securing special adaptive equipment for patients.
• Maintain and submit necessary documentation adhering to required standards and regulations.
• Supervise Physical Therapist Assistants and Aides in patient care activities.
• Conduct and participate in staff development programs and meetings as needed.

Requirements of the Physical Therapist

• PT License (NC)
• Home health experience with HCHB experience
• Oasis knowledge
