Job Title: Physical Therapist I/Physical Therapist W/E/Physical Therapist II Profession: Physical Therapist Specialty: - Duration: 11 months Shift: Day Hours per Shift: 40 Experience: 2+ years minimum License: New Jersey Physical Therapy License Certifications: DPT Must-Have: - CPR/BLS certification Description: The Physical Therapist provides physical therapy services to individuals and their families in collaboration with other health care providers. Performs all functions associated with the evaluation of patients across all age groups. Executes therapeutic treatment for patients of all age groups. Completes all required documentation within the designated timeline. Lifts a minimum of 50 lbs., pushes and pulls a minimum of 100 lbs., and stands for a minimum of 6 hours a day. Identifies the needs of the patient population served and modifies care accordingly. Communicates effectively with patients, parents, and primary caregivers at their respective levels. Adheres to established organizational competencies. Possesses a strong foundation of medical knowledge and technical abilities to assess and treat patients effectively. Demonstrates extensive skills in comprehensive musculoskeletal assessment and therapeutic exercise prescription. Utilizes manual therapy techniques and gait and balance training. Conducts neuromuscular re-education and functional mobility training. Offers pain management interventions and orthopedic rehabilitation. Provides neurological rehabilitation and post-operative rehabilitation services. Conducts fall risk assessments and prevention strategies. Assesses and trains patients on assistive devices such as walkers, canes, and crutches. Performs range of motion and strength testing. Trains patients in transfer and mobility techniques. Develops individualized treatment plans based on assessment results. Administers and interprets outcome measures. Documents in Electronic Medical Records (EMR), such as Epic. Educates patients and caregivers regarding their care plans. Engages in discharge planning and the development of home exercise programs. Ensures compliance with infection prevention and safety standards. Stays informed about evidence-based rehabilitation practices. Complies with regulatory standards and interdisciplinary care coordination.
