Description:
Provides patient care assessment and treatment.
Performs initial evaluations.
Assesses patients’ motor and developmental skills to determine their deficits.
Evaluates patients’ physical and mental abilities and analyzes data to develop realistic rehabilitation plans.
Reviews physician referrals and patients’ medical records to help determine diagnoses and required treatments.
Records prognosis, treatment, response, and progress in patients’ medical records.
Prepares a variety of written materials, including evaluations, plans of care, progress notes, and correspondence related to patient care.
Maintains patient records confidentially.
Facilitates patient mobility skills, including the use of safe patient handling equipment.
Manages equipment utilized in treatments.
Adapts and modifies environments, equipment, and materials to assist with functional mobility and alleviate deficits.
Documents and identifies progress and plans for reevaluation.
Evaluates treatment effects and adjusts treatment to maximize benefits.
Facilitates social adjustment and prevents physical or mental disability.
Evaluates patients in a comprehensive, age-appropriate manner.
Sets quantifiable and realistic goals.
Uses advanced judgment and skills in implementing treatment plans.
Communicates with physicians timely, and keeps documentation current.
Accomplishes patient mobility using appropriate safe patient handling techniques.
Maintains up-to-date cumulative physical therapy records.
Facilitates the provision of excellent customer service.
Takes initiative to resolve client concerns.
Educates patients and families about treatment procedures to continue at home.
Instructs patients, educational personnel, and families to facilitate skills in physical functioning and health maintenance.
Trains caregivers and patients how to provide for needs during and after therapy.
Promotes activities that help patients learn mobility skills within their capabilities.
Implements programs and activities to assist patients in learning mobility skills and adjusting to limitations.
Recommends changes in patients’ work or living environments according to their needs.
Works collaboratively in a team.
Plans, coordinates, and provides training to families, patients, and medical professionals.
Provides clinical educational opportunities for students and team members.
Supervises physical therapist assistants and other team members.
Consults with rehabilitation teams to select and coordinate physical therapy and other therapeutic activities.
Assists peer physical therapists, physical therapist assistants, and other team members with workload as needed.
Communicates departmental information effectively.
Collaborates with other healthcare team members in a multidisciplinary manner.
Assists in the orientation of new employees.
