Key Responsibilities of the Physical Therapist (Home Health)- Conduct higher-level visits such as SOC, evaluations, reassessments, and discharges
- Document visits within various EMR systems including Kantime, Kinnser (WellSky), Axxess, and Homecare Homebase
- Coordinate visit scheduling directly with patients
- Provide care across multiple home health agency environments
- Utilize central scheduling platform: Pariox EMR
Requirements of the Physical Therapist (Home Health)- Valid Physical Therapy license in the state of Florida
- Experience in home health settings
- Proficiency in EMR systems
