Travel PT Home Health jobs in Sacramento, CA covering Sacramento and Stockton offer 13-week contracts with Monday through Friday 8-hour days and 40 hours expected weekly. You must have a California license and home health experience. On-call duties are required.
Your responsibilities include performing physical therapy interventions, evaluations, and diagnostic tests in patients’ homes. You will document initial evaluations, treatment plans, and re-evaluate patient progress every thirty days, updating care plans as needed
