Travel Physical Therapist (PT) - $2,751 per week in Pleasant Hill, CA

Periodic office meetings are required. Must have reliable transportation and a current driver's license. Agency-required liability insurance is necessary. Average number of patients per day is 5-6. Contractor will be assigned between 5-10 patients per day which equate to roughly 6-8 points. Guaranteed hours are not provided. If the contractor cannot meet financial needs due to low patient census, they should discuss floating to another clinic. Types of cases include Start of Care and Follow Ups. First-time travelers are not accepted; recent Home Health experience is required. Weekend requirements vary per location and should be discussed during the interview. On-call requirements vary per job/location and should be discussed with the manager. Must be willing to travel up to 75 miles from the home clinic. Mileage will be reimbursed at the IRS standard rate. Commuting to and from the first and last patient is not billable. Local candidates are accepted at the same rate. Pending licenses are not accepted; must have a license in hand unless it takes a few days to obtain. There is a need for a contractor due to increased workload and volume. Weekly travel mileage is estimated at 200 miles. Weekly productivity expectations are 30 points. Experience with HomeCare HomeBase EMR is required. Home health experience is necessary. Candidates should confirm their awareness of the job requirements related to home health assignments. All documentation must be completed within 24 hours of the patient appointment. There are no guaranteed hours with this contract. Contractors must hit weekly productivity points of 30, which equates to 40 hours per week. Any missed patient opportunities may result in cancelation.
