Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment based on physician orders/referrals
• Complete comprehensive evaluations using appropriate tools and procedures
• Establish measurable goals and develop treatment plans to enhance patient function
• Re-assess treatment plan effectiveness and progress towards goals regularly
• Communicate patient needs and progress to the team, family, and other relevant parties
• Consult with medical providers regarding patient progress and discharge plans
• Recommend and assist in securing adaptive equipment for rehabilitation programs
• Maintain and submit required documentation as per regulations and policies
• Adhere to the Company's Code of Business Conduct and compliance programs
• Supervise Physical Therapist Assistants and Aides in patient care activities
• Participate in staff development and maintain clinical practice skills

Requirements of the Physical Therapist

• Active NC PT license
• Experience in Home Health PT, Oasis, and HCHB
