Travel Physical Therapist (PT) - $2,770 per week in Garden City, NY

Job Title: Physical Therapist - Home Health Profession: Therapy/Rehabilitation Specialty: PT Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year of Adult Home Health Experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Recent Home Health experience Description: This position involves providing home health therapy services through home visits. Periodic office meetings are required. Candidates must have reliable transportation, a current driver's license, and agency-required liability insurance. The average number of patients per day will range from 5-6. Contractors will be assigned between 5-10 patients per day, which equates to roughly 6-8 points. Productivity expectations are a minimum of 30 points per week. Each type of visit and activity, including documentation and travel, holds a different point value. In the event of low patient census affecting financial needs, contractors can discuss floating to a nearby clinic to pick up hours. Types of cases include Start of Care and Follow Ups. This position requires the clinician to be willing to travel up to 75 miles from the home clinic. Mileage will be reimbursed at the IRS standard rate. Travelers will be compensated for the distance traveled from one patient’s home to another. The commute to and from the first and last patient is not billable. Weekend requirements vary per location and profession, and these should be discussed during the interview process. Travelers will have the opportunity to float to another clinic if there are not enough patients to meet their financial needs. Candidates must be comfortable accepting all scheduled patients, regardless of distance from previous appointments. All documentation must be completed within 24 hours of patient appointments, as delayed documentation can affect billing and follow-up care. There are no guaranteed hours with this contract, and all overtime must be pre-approved. Candidates should be open to on-call rotations as required by the position.
