This home health Physical Therapist role is based in Hayward, California, in the heart of the East Bay. In this position, you will provide one-on-one, in-home physical therapy services to adult and geriatric patients recovering from illness, surgery, or managing chronic conditions. You will perform comprehensive in-home evaluations, develop individualized plans of care, and deliver evidence-based interventions focused on functional mobility, strength, balance, fall prevention, and home safety. Your work will emphasize helping patients maintain independence, reduce rehospitalizations, and achieve meaningful goals within their own homes. You will collaborate closely with an interdisciplinary team that may include nurses, occupational therapists, speech therapists, social workers, and physicians. Communication and care coordination are key, as you will regularly participate in case conferences, update care plans, and provide clear feedback to the broader care team. The role offers the chance to build strong therapeutic relationships, as you follow patients over time and see the direct impact of your interventions in their daily environments. A typical day includes driving within a defined territory in and around Hayward to visit patients in their homes, assisted living residences, or independent living communities. You will schedule and manage your own patient visits within organizational guidelines, typically seeing multiple patients per day depending on acuity and visit type. Visits may include initial evaluations, routine treatment sessions, reassessments, and discharge visits. Patient caseloads are generally focused on adult and geriatric populations with a variety of diagnoses such as orthopedic conditions, post-operative care, neurologic disorders, cardiopulmonary disease, and general deconditioning. You will complete all documentation in an electronic medical record system, often using a laptop or tablet in the field. Timely documentation, clear goal-setting, and measurable outcomes are emphasized, with support from clinical leadership to ensure you can maintain both quality patient care and compliance with regulatory requirements. You will also provide ongoing education to patients and caregivers on home exercise programs, safe mobility techniques, use of assistive devices, and environmental modifications to reduce fall risk.
