Travel Physical Therapist (PT) - $2,813 per week in Sunnyvale, CA

Job Title: Physical Therapist Profession: Rehabilitation Therapy Specialty: Physical Therapist Duration: 13 weeks Shift: Days Hours per Shift: 8:00 am - 5:30 pm (Schedule can be flexible based on patient's availability.) One weekend required. Experience: Minimum of 1 year of Home Health experience License: Valid Physical Therapy License Certifications: N/A Must-Have: Home Health experience Description: The primary goal of the Physical Therapist is to restore function and minimize disability following disease or injury. The therapist helps clients reach their maximum performance and function within their capabilities. They will evaluate and treat patients using prescribed physical therapy programs. The Physical Therapist may supervise Physical Therapy Assistants. Initial admission and evaluation visits may be conducted by the therapist. They will notify the assigned Clinical Team Manager of relevant scores within established timelines. The ideal candidate will document observations, clinical findings, problems, skilled interventions, treatments, goals, and discharge plans. This individual will also perform other duties as assigned in alignment with the goals and mission of the organization. Territory details may include various neighboring cities as needed.
