Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment of patients based upon physician orders/referrals
• Complete comprehensive evaluations using appropriate tools and procedures
• Review patient records for current diagnosis, past medical history, precautions, and contraindications
• Establish measurable goals and develop treatment plans
• Regularly reassess treatment plan effectiveness and patient progress
• Communicate patient needs and progress to the treatment team and relevant parties
• Consult with medical providers regarding patient evaluation, treatment, and discharge
• Recommend and assist in securing special adaptive equipment
• Maintain and submit all required documentation timely and accurately
• Adhere to all applicable standards and regulations
• Supervise Physical Therapist Assistants and Aides
• Participate in staff development programs and maintain clinical practice skills

Requirements of the Physical Therapist

• Must have a valid Physical Therapy license
• Proficiency in Home Health
• Experience with Home Care Home Base documentation
