Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment based on physician referrals
• Complete comprehensive evaluations using appropriate tools and procedures
• Review patient records for diagnosis and medical history
• Establish goals and develop treatment plans
• Regularly reassess treatment plans and modify as necessary
• Communicate patient needs and progress to relevant parties
• Consult with medical providers on patient evaluations and treatment progress
• Recommend and assist with adaptive equipment for patient rehabilitation
• Maintain and submit required documentation
• Adhere to professional and legal standards
• Supervise Physical Therapist Assistants and Aides
• Participate in and conduct staff development programs and meetings

Requirements of the Physical Therapist

• Licensed NC PT or compact
• Home health experience
• Experience with HCHB and OASIS
