Travel Physical Therapist (PT) - $2,939 per week in Oakland, CA

Required to supervise Physical Therapy Assistants. Next day mandatory reporting of start of care visits. Clinical notes and other documentation are due within 48 hours of the visit. Maintain timely communication with supervisor, intake, and office. Be generally responsive during normal business hours. Territory will be required to expand as needed. Schedule consists of 40 hours per week. Training includes 3 full days, one in-house and two in the field. Documentation system is Kinnser. Equipment issued includes an iPad and Verizon One Talk. Mileage reimbursement will be provided. A company car option is available with a decreased bill rate. Productivity requirement is 30 points per week. Caseloads and visit mixes per day will equal an average of 8 hours per day and 40 hours per week. Overtime must be pre-approved by the Clinical Supervisor. Visit types include start of care, recertification, resumption of care, OASIS discharge, evaluation, revisit, and NON OASIS discharge. Point values will vary by visit type.
