In this role, you will provide skilled physical therapy services to adult and geriatric patients in their homes throughout Roseburg and the surrounding area. Your typical day will include performing comprehensive evaluations, developing individualized plans of care, and delivering hands-on treatment focused on improving functional mobility, strength, balance, and safety in the home environment. You will educate patients and caregivers on home exercise programs, fall prevention strategies, safe transfers, and the proper use of assistive devices.
You will manage a community-based caseload that may include patients recovering from surgery, stroke, cardiac events, fractures, and those living with chronic conditions such as COPD, heart failure, or neurologic diagnoses. You will travel between homes within a defined geographic territory around Roseburg, allowing you to become familiar with the community while minimizing extensive driving.
