We are seeking an experienced Physical Therapist to join our team.
As a Physical Therapist, you will be responsible for assessing and treating patients to help them regain physical function and improve mobility.
You will work within a collaborative healthcare team and design individualized treatment plans that promote independence and enhance the quality of life for residents.
Key Responsibilities:
Perform thorough evaluations to assess patients' physical abilities and develop personalized treatment plans.
Provide physical therapy interventions to improve strength, mobility, and functional abilities.
Educate patients and their families on therapeutic exercises, safe movement techniques, and home modifications.
Collaborate with the interdisciplinary team to coordinate care and optimize patient outcomes.
Monitor patient progress and adjust treatment plans as necessary to ensure the best possible outcomes.
Maintain accurate and timely documentation of patient assessments, progress notes, and treatment plans in accordance with facility policies.
Ensure that care plans align with regulatory requirements and best practices in physical therapy.
Qualifications:
Experience working in a skilled nursing facility or similar healthcare setting is preferred.
Knowledge of physical therapy techniques, therapeutic modalities, and adaptive equipment is essential.
Excellent communication, interpersonal, and problem-solving skills are required.
Ability to work both independently and collaboratively in a team environment is necessary.
A compassionate, patient-focused approach to care is essential.
