Key Responsibilities of the Physical Therapist- Perform physical therapy evaluation and treatment of patients based on physician orders/referrals
- Complete comprehensive evaluations using appropriate evaluation and assessment tools
- Review patient records for current diagnosis, medical history, precautions, and contraindications
- Establish measurable goals and develop treatment plans for patient independence and function
- Regularly reassess treatment plan effectiveness and adjust as needed
- Communicate patient needs and progress to the treatment team and involved parties
- Consult with medical providers regarding patient evaluation progress and discharge
- Recommend and assist in securing special adaptive equipment for patient improvement
- Maintain and submit necessary documentation according to regulations
- Supervise Physical Therapist Assistants and Aides as per state practice act
- Attend and participate in staff development programs and maintain clinical skills
Requirements of the Physical Therapist- WV PT license (compact licenses accepted)
- Experience with HCHB preferred, OASIS, and home health
