Key Responsibilities

• Evaluate and treat patients in a rehabilitation and wellness setting
• Develop and implement individualized physical therapy treatment plans
• Monitor and document patient progress throughout the course of care
• Collaborate with interdisciplinary team members to support patient outcomes
• Maintain valid state-issued photo identification and wear a photo badge at all times while on facility premises

Required Qualifications

• Active Physical Therapist (PT) license in the state of California
• Graduate of an accredited Physical Therapy program
• Ability to fulfill a 15-week contract assignment
• Must present a valid state or federal issued photo identification upon arrival at the facility

Preferred Experience / Skills

• Prior experience in a rehabilitation or wellness setting
• Strong documentation and clinical assessment skills
