Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
• Conduct 60-minute evaluations and treatment sessions in an outpatient setting
• Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
• Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
• Serve as an educator for patients, families, students, and coworkers; participate in student programs as assigned with appropriate supervision and development of learning objectives
• Adhere to discipline-specific State Practice Act and all applicable regulatory requirements, maintaining professional licensure in good standing

Required Qualifications

• Active Physical Therapist license in the state of California (in good standing)
• Outpatient physical therapy experience required; new graduates will not be considered
• Demonstrated ability to perform comprehensive patient evaluations and develop goal-oriented treatment plans
• Knowledge of and adherence to state and federal documentation and compliance standards

Preferred Experience / Skills

• Experience with functional quality indicator assessments and letters of medical necessity
• Prior experience supervising or mentoring PT students in a clinical setting
• Active involvement in continuing education and professional development activities
