Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
Conduct 60-minute initial evaluations and 30-minute follow-up appointments in an outpatient setting
Complete accurate and timely medical record documentation including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
Serve as an educator for patients, families, students, and coworkers; participate in student programs as assigned with appropriate supervision and evaluation
Adhere to the discipline-specific State Practice Act and all applicable regulatory requirements, maintaining professional licensure in good standing
Required Qualifications
Active and unrestricted Physical Therapist license in the state of Colorado
Experience performing comprehensive patient evaluations and developing evidence-based treatment plans
Proficiency in medical record documentation in compliance with state, federal, and insurance guidelines
Knowledge of and adherence to discipline-specific professional codes of conduct and ethical standards
Commitment to patient and employee safety practices including safe patient handling and infection control protocols
Preferred Experience / Skills
Prior experience in an outpatient physical therapy setting
Experience supervising or mentoring physical therapy students
Active participation in continuing education and professional development activities
