Key Responsibilities
  • Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
  • Conduct 60-minute initial evaluations and 30-minute follow-up sessions in an outpatient setting
  • Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
  • Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
  • Serve as an educator for patients, families, students, and coworkers; participate in student programs as assigned with appropriate supervision and evaluation
  • Adhere to the discipline-specific State Practice Act and all regulatory requirements; maintain professional licensure in good standing and fulfill continuing education requirements
Required Qualifications
  • Active and unrestricted Physical Therapist licensure in the state of Colorado
  • Demonstrated ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
  • Proficiency in clinical documentation including goal writing, functional assessments, and compliance with state and federal guidelines
  • Knowledge of and adherence to discipline-specific professional codes of conduct and ethical standards
  • Commitment to patient and employee safety practices
Preferred Experience / Skills
  • Prior experience in an outpatient physical therapy setting
  • Experience supervising or mentoring physical therapy students
  • Familiarity with performance improvement methodologies and organizational quality initiatives
