    Contract | Days | 13 Weeks | Monday–Wednesday 7:00am–5:00pm, Tuesday–Thursday 7:00am–4:00pm, Friday 7:00am–1:00pm | Outpatient | Onsite

    Key Responsibilities
    • Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction.
    • Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines.
    • Delegate and direct all discipline-specific therapy services for assigned patients, ensuring design of functional treatment programs that support effective return to community activities.
    • Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing appropriate supervision, developing learning objectives, and completing student evaluations.
    • Adhere to discipline-specific State Practice Act and all regulatory agency requirements, maintaining professional licensure in good standing and completing all state-mandated continuing education requirements.
    • Support organizational performance improvement initiatives by identifying opportunities to improve quality of services and providing feedback through appropriate channels.
    Required Qualifications
    • Active and unrestricted Physical Therapist license in the state of Georgia.
    • Ability to complete comprehensive patient evaluations and develop evidence-based treatment plans with appropriate goals and discharge planning.
    • Proficiency in medical record documentation in compliance with state, federal, and payer guidelines.
    • Knowledge of and adherence to discipline-specific professional code of conduct and ethical standards.
    • Demonstrated commitment to patient and employee safety practices.
    Preferred Experience / Skills
    • Outpatient orthopedic physical therapy experience strongly preferred.
    • Experience supervising or mentoring physical therapy students in a clinical setting.
    • Active involvement in continuing education and professional development activities.
