    Key Responsibilities
    • Conduct comprehensive patient evaluations and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
    • Perform 60-minute initial evaluations and 30-minute follow-up sessions in an outpatient setting
    • Complete accurate and timely medical record documentation including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
    • Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support return to community activities
    • Serve as an educator for patients, families, students, and coworkers; participate in student supervision programs as assigned
    • Adhere to the applicable State Practice Act and all regulatory requirements, maintaining licensure in good standing and fulfilling continuing education obligations
    Required Qualifications
    • Active and unrestricted Physical Therapist license in the state of Colorado
    • Graduate of an accredited Physical Therapy program
    • Experience performing comprehensive PT evaluations and developing evidence-based treatment plans
    • Ability to document clinical care accurately and in compliance with state, federal, and insurance guidelines
    • Commitment to patient and employee safety standards and safe patient handling practices
    Preferred Experience / Skills
    • Prior experience in an outpatient physical therapy setting
    • Experience supervising or mentoring PT students
    • Familiarity with performance improvement methodologies and quality initiatives
