    Contract | Days | Five 8-hour shifts | Monday–Friday with 2 weekends per month

    Key Responsibilities
    • Completes comprehensive evaluations of assigned patients and develops, implements, and modifies individualized treatment plans in accordance with patient needs and physician direction
    • Collaborates with the interdisciplinary team to address all barriers to discharge, including equipment trialing and ordering, patient and family training, home exercise program issuance, and planning for continuing services
    • Completes medical record documentation in a clear, concise, accurate, and timely manner, including daily notes, goal writing, functional quality indicator assessments, letters of medical necessity, discharge documentation, and charge entry in compliance with state, federal, and intermediary guidelines
    • Manages individual, concurrent, and group treatment sessions effectively based on patient care needs and demonstrates flexibility in caseload management
    • Participates in student programs as assigned, providing supervision, developing learning objectives, and completing student evaluations
    • Adheres to discipline-specific State Practice Act and all regulatory requirements; maintains active professional licensure in good standing and completes all state-mandated continuing education requirements
    Required Qualifications
    • Active Physical Therapist license in good standing in the state of Ohio
    • Ability to complete comprehensive patient evaluations and develop clinically sound treatment plans and goals
    • Proficiency in medical record documentation in compliance with state, federal, and intermediary guidelines
    • Commitment to patient and employee safety standards and safe patient handling practices
    Preferred Experience / Skills
    • Experience working within an interdisciplinary team environment in an inpatient or acute rehabilitation setting
    • Familiarity with functional quality indicator assessments and letters of medical necessity
    • Active involvement in professional development, continuing education, and professional organizations
