Key Responsibilities- Evaluate and treat patients in a skilled nursing facility providing both long-term care and short-term rehabilitation services
- Develop and implement individualized physical therapy treatment plans to maximize patient functional outcomes
- Document patient progress, treatment plans, and discharge summaries in accordance with facility and regulatory standards
- Collaborate with interdisciplinary care teams to coordinate patient care and ensure continuity of services
- Participate in weekend rotation coverage as part of a structured scheduling model
Required Qualifications- Active Physical Therapist (PT) license in the state of Maryland or eligibility to obtain
- Graduate of an accredited Physical Therapy program
- Ability to work a full-time schedule of 40 hours per week
- Willingness to participate in a weekend rotation
Preferred Experience / Skills- Prior experience in a skilled nursing, long-term care, or short-term rehabilitation setting
- Familiarity with Medicare and insurance documentation requirements in a SNF environment
- Strong interpersonal and communication skills for working within a multidisciplinary team
