Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in collaboration with physician direction and the interdisciplinary team.
Manage individual, concurrent, and group treatment sessions effectively, ensuring the plan of care is communicated, progressed, and aligned with discharge goals throughout the patient's length of stay.
Complete all medical record documentation accurately and in a timely manner, including daily notes, goal writing, functional quality indicator assessments, letters of medical necessity, discharge documentation, and charge entry in compliance with state, federal, and intermediary guidelines.
Provide patient and family education, including home exercise program instruction, equipment trialing and ordering, and planning for continuation of care services.
Participate in student clinical programs as assigned, providing supervision, developing learning objectives, and completing student evaluations.
Actively engage in professional development, performance improvement initiatives, and patient advocacy efforts including community events and professional organization involvement.
Required Qualifications
Active and unrestricted Physical Therapist license in the state of Ohio in good standing.
Adherence to the Ohio Physical Therapy Practice Act and all applicable regulatory agency requirements.
Demonstrated competency in clinical reasoning, treatment planning, and caseload management across patient populations.
Ability to complete thorough and compliant medical record documentation in accordance with hospital policies and state and federal guidelines.
Completion of all state-mandated continuing education requirements to maintain licensure.
Preferred Experience / Skills
Experience working within an inpatient rehabilitation or acute care hospital setting.
Familiarity with interdisciplinary team collaboration and discharge planning processes.
Prior experience supervising or mentoring physical therapy students in a clinical setting.
