Key Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans
• Provide therapeutic interventions to improve patient mobility, strength, and functional independence
• Document patient progress and adjust treatment plans as clinically appropriate
• Collaborate with interdisciplinary care team to support patient rehabilitation goals
• Deliver care within a skilled nursing and rehabilitation setting

Required Qualifications

• Active Physical Therapist (PT) license in the state of Montana
• Graduate of an accredited Physical Therapy program
• Current CPR/BLS certification
• Experience providing physical therapy in a skilled nursing or rehabilitation environment

Preferred Experience / Skills

• Prior experience in a long-term care or post-acute care setting
• Strong documentation and communication skills
