Key Responsibilities
Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
Delegate and direct all discipline-specific therapy services for assigned patients, ensuring design of functional treatment programs that support effective return to community activities
Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing appropriate supervision, developing learning objectives, and completing student evaluations
Adhere to discipline-specific State Practice Act and all applicable regulatory agency requirements, maintaining professional licensure in good standing
Actively participate in professional development through continuing education, self-study, and involvement in professional activities and organizations
Required Qualifications
Active Physical Therapist licensure in good standing in the state of Minnesota
Prior clinical experience as a Physical Therapist required; new graduates will not be considered
Demonstrated competency in patient evaluation, treatment planning, and goal writing
Knowledge of and adherence to state and federal documentation and compliance guidelines
Commitment to patient and employee safety standards and safe patient handling practices
Preferred Experience / Skills
Experience in an outpatient physical therapy setting
Familiarity with performance improvement methodologies and quality of care initiatives
Experience supervising or mentoring Physical Therapy students
