    Key Responsibilities
    • Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in alignment with patient needs and physician direction.
    • Collaborate with the interdisciplinary team to address all barriers to discharge, including equipment trialing and ordering, patient and family training, home exercise program issuance, and coordination of continuing services.
    • Complete medical record documentation in a clear, concise, accurate, and timely manner, including daily notes, goal writing, functional quality indicator assessments, letters of medical necessity, discharge documentation, and charge entry in compliance with state, federal, and intermediary guidelines.
    • Manage individual, concurrent, and group treatment sessions effectively based on patient care needs, demonstrating flexibility in caseload management.
    • Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations.
    • Adhere to discipline-specific state practice acts and regulatory requirements, maintaining professional licensure in good standing and completing all mandated continuing education requirements.
    Required Qualifications
    • Active Physical Therapist license in good standing in the state of New Jersey.
    • Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans.
    • Knowledge of and compliance with state, federal, and intermediary documentation and billing guidelines.
    • Demonstrated commitment to patient and employee safety practices.
    Preferred Experience / Skills
    • Experience in an inpatient hospital or rehabilitation setting.
    • Familiarity with interdisciplinary team collaboration and discharge planning processes.
    • Experience supervising or mentoring Physical Therapy students.
