Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in alignment with patient needs and physician direction.
Collaborate with the interdisciplinary team to address all barriers to discharge, including equipment trialing and ordering, patient and family training, home exercise program issuance, and coordination of continuing services.
Complete medical record documentation in a clear, concise, accurate, and timely manner, including daily notes, goal writing, functional quality indicator assessments, letters of medical necessity, discharge documentation, and charge entry in compliance with state, federal, and intermediary guidelines.
Manage individual, concurrent, and group treatment sessions effectively based on patient care needs, demonstrating flexibility in caseload management.
Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations.
Adhere to discipline-specific state practice acts and regulatory requirements, maintaining professional licensure in good standing and completing all mandated continuing education requirements.
Required Qualifications
Active Physical Therapist license in good standing in the state of New Jersey.
Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans.
Knowledge of and compliance with state, federal, and intermediary documentation and billing guidelines.
Demonstrated commitment to patient and employee safety practices.
Preferred Experience / Skills
Experience in an inpatient hospital or rehabilitation setting.
Familiarity with interdisciplinary team collaboration and discharge planning processes.
Experience supervising or mentoring Physical Therapy students.
