Key Responsibilities

• Perform Start of Care evaluations for home health patients in the Norwalk and Westport service areas
• Provide skilled physical therapy services to patients in a home health setting within an approximate 40-mile radius
• Complete clinical documentation accurately and timely using HCHB electronic medical records system
• Complete OASIS assessments in accordance with home health regulatory requirements
• Meet productivity expectations of 17 points per week

Required Qualifications

• Active Physical Therapist (PT) license in the state of Connecticut
• Prior home health experience required
• Experience with HCHB (Homecare Homebase) EMR required
• Experience completing OASIS assessments required

Preferred Experience / Skills

• Experience performing Start of Care evaluations in a home health environment
• Familiarity with the Norwalk and Westport, CT geographic areas
