Key Responsibilities

• Conduct comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
• Complete 60-minute evaluations and 30-minute follow-up sessions; maintain clear, concise, and timely medical record documentation including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry
• Design functional treatment programs focused on progressing patients toward discharge and effective return to community activities
• Delegate and direct discipline-specific therapy services for assigned patients; ensure care is provided under valid physician orders and within insurance approvals
• Serve as an educator for patients, families, students, and coworkers; participate in student programs as assigned, including supervision, learning objective development, and student evaluations
• Adhere to discipline-specific state practice acts, regulatory requirements, and professional codes of conduct; uphold patient and employee safety standards at all times

Required Qualifications

• Active and unrestricted Physical Therapist (PT) license in the state of Colorado
• Completion of all state-mandated continuing education requirements
• Demonstrated competency in clinical reasoning, treatment planning, and functional goal writing
• Ability to maintain compliance with state, federal, and intermediary documentation and billing guidelines

Preferred Experience / Skills

• Prior experience in an outpatient physical therapy setting
• Experience supervising or mentoring PT students
• Familiarity with performance improvement methodologies and quality initiatives
