Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
Adhere to discipline-specific State Practice Act and all regulatory agency requirements; maintain professional licensure in good standing and fulfill all state-mandated continuing education requirements
Actively participate in performance improvement initiatives and contribute feedback through staff meetings, focus groups, or direct supervision to maximize outcomes
Required Qualifications
Active and unrestricted Physical Therapist license in the state of Virginia
Minimum of 2 years of clinical experience as a Physical Therapist (new graduates will not be considered)
Experience in an outpatient setting with demonstrated clinical reasoning and treatment planning skills
Ability to complete thorough and compliant medical record documentation in accordance with state, federal, and payer guidelines
Commitment to patient and employee safety standards and adherence to professional codes of conduct
