Schedule / Job TypeContract | 13-Week Assignment | Skilled Nursing Facility (SNF)
Key Responsibilities- Evaluate patients and develop individualized physical therapy treatment plans within a skilled nursing facility setting
- Implement therapeutic interventions to improve patient mobility, strength, and functional independence
- Monitor and document patient progress, adjusting treatment plans as clinically appropriate
- Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
- Educate patients and families on home exercise programs and safe functional techniques
Required Qualifications- Active Physical Therapist (PT) license in the state of Wyoming or eligibility for licensure
- Graduate of an accredited Physical Therapy program
- Current CPR/BLS certification
- Experience working in a skilled nursing facility or similar post-acute care environment
Preferred Experience / Skills- Prior experience in a SNF or long-term care setting
- Familiarity with MDS documentation and Medicare guidelines in a post-acute environment
