Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans and goals focused on progressing patients toward discharge within an established length of stay.
Collaborate with the interdisciplinary team to address all barriers to discharge, including equipment trialing and ordering, patient and family training, home exercise program issuance, and planning for continuing services.
Complete medical record documentation in a clear, concise, accurate, and timely manner, including daily notes, goal writing, functional quality indicator assessments, letters of medical necessity, discharge documentation, and charge entry in compliance with state, federal, and intermediary guidelines.
Effectively manage individual, concurrent, and group treatment sessions based on patient care needs while demonstrating flexibility in caseload management.
Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations.
Adhere to discipline-specific state practice acts and regulatory requirements, maintaining professional licensure in good standing and completing all state-mandated continuing education requirements.
Required Qualifications
Active and unrestricted Physical Therapist license in the state of Ohio.
Demonstrated ability to perform comprehensive patient evaluations and develop evidence-based treatment plans.
Proficiency in medical record documentation in compliance with state, federal, and payer guidelines.
Knowledge of and adherence to discipline-specific professional codes of conduct and applicable state practice acts.
Ability to collaborate effectively within an interdisciplinary team environment.
Preferred Experience / Skills
Experience in an inpatient rehabilitation or acute care hospital setting.
Familiarity with functional quality indicator assessments and letters of medical necessity documentation.
Experience supervising or mentoring Physical Therapy students in a clinical setting.
