    Key Responsibilities
    • Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans and goals focused on progressing patients toward discharge within an established length of stay.
    • Collaborate with the interdisciplinary team to address all barriers to discharge, including equipment trialing and ordering, patient and family training, home exercise program issuance, and planning for continuing services.
    • Complete medical record documentation in a clear, concise, accurate, and timely manner, including daily notes, goal writing, functional quality indicator assessments, letters of medical necessity, discharge documentation, and charge entry in compliance with state, federal, and intermediary guidelines.
    • Effectively manage individual, concurrent, and group treatment sessions based on patient care needs while demonstrating flexibility in caseload management.
    • Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations.
    • Adhere to discipline-specific state practice acts and regulatory requirements, maintaining professional licensure in good standing and completing all state-mandated continuing education requirements.
    Required Qualifications
    • Active and unrestricted Physical Therapist license in the state of Ohio.
    • Demonstrated ability to perform comprehensive patient evaluations and develop evidence-based treatment plans.
    • Proficiency in medical record documentation in compliance with state, federal, and payer guidelines.
    • Knowledge of and adherence to discipline-specific professional codes of conduct and applicable state practice acts.
    • Ability to collaborate effectively within an interdisciplinary team environment.
    Preferred Experience / Skills
    • Experience in an inpatient rehabilitation or acute care hospital setting.
    • Familiarity with functional quality indicator assessments and letters of medical necessity documentation.
    • Experience supervising or mentoring Physical Therapy students in a clinical setting.
